45세 녀, 주부 불만 사항: 이 환자는 5개월 전 허리디스크 수술 이후 2주 만에 요추 5-천골 1을 재발했습니다. MRI에서 2mm 디스크 돌출이 확인되었습니다. 환자는 2번의 경막외 주사와 가바펜틴을 받았지만 큰 완화 효과를 얻지 못했습니다. 2개월 전 통증이 매우 심하여 환자는 8일간 침대에 누웠습니다. 경막외 주사를 맞고 2일 후 걷기 시작할 수 있었습니다. 통증을 완화할 수 있는 유일한 자세는 왼쪽 측면에 누워 다리 사이에 베개를 두는 것입니다. 환자의 오른쪽 좌골신경통은 하루가 끝날수록 악화되었습니다.

Complaints: The patient re-herniated her lumbar 5 – sacral 1 two weeks after a discectomy performed five months ago. An MRI showed a 2 mm disc protrusion. The patient had two epidurals and Gabapentin, but they did not provide much relief. Two months ago, the pain was so severe that the patient was bedridden for eight days. She was able to walk two days after the epidural. The only position that eliminated pain was lying down on the left side with a pillow in between the legs. The patient’s right sciatica worsened by the end of the day.

Treatment: The patient has received ten sessions of acupuncture twice a week so far. The pain was noticeably less immediately after the first treatment. Each week, there was further relief in terms of intensity, frequency, and duration. By the 10th visit, the patient was able to walk for over one mile.
논의: 디스크 탈출증이 3mm 이하인 환자들에게 더 나은 통증 완화 가능성이 있습니다. 올바른 침술 지점의 조합에 더해, 환자는 구조적 문제가 적어야 하고 침술 자극에 대한 반응성이 높아야 합니다.
Discussion: There is potential for better relief in patients with a disc herniation of 3 mm or less. In addition to a correct combination of acupuncture points, the patient must have a less structural problem and high reactivity to acupuncture stimulation.
